The service is based in a beautiful, rural location and has 4 bedded residential service. 

The service has been designed for 4 users with complex needs. 

the Role:

As a Care Coordinator, you will be responsible for overseeing and coordinating the care services provided to our clients. Your role will involve liaising with healthcare professionals, clients, and their families to ensure the delivery of high-quality care. It will also include Managing Care Rotas and Payroll.

  • Assessing clients’ care needs and developing personalised care plans
  • Coordinating and scheduling care services for clients
  • Monitoring and evaluating the effectiveness of care plans
  • Managing and Coordinating the weekly rotas
  • Completing weekly payroll (data input)
  • Ensuring compliance with regulatory standards and guidelines
  • Maintaining accurate and up-to-date client records

Required: 

  • Previous experience in a care coordination or similar role is desirable

  • Observing, coaching and giving feedback to colleagues who are working towards their Care Certificate on how well they demonstrate the required skills, knowledge and behaviours.
  • Coaching and mentoring new colleagues.
  • Demonstrating practice leadership.
  • Monitoring the levels of Person-Centred Active Support (PCAS) being demonstrated and looking for ways we can continually improve.
  • Leading team or house meetings
  • Involvement in ‘Voices to be Heard’.
  • Working with families and other circles of support.
  • Working with healthcare professionals and external agencies to ensure that the needs and interests of the people we support are met and upheld.

Required:

  • Qualified to diploma level 3 minimum (level 5 is ideal but not essential, however you will need to study towards this). 
  • Solid experience as a Service Manager (ideally registered but this is not essential)
  • Full driving licence and own vehicle
  • Solid people management and performance management experience
  • Experience working within the CQC framework
  • Learning disabilities background
